West Yorkshire Health and Care Partnership have published ‘Your Guide to a Disciplinary Investigation’, which answerThe cover image of the disciplinary guide.s many of the questions that employees ask when they are being taken through a disciplinary process.

The guide, developed by NHS Wales, has been adapted for thousands of employees working across the NHS in West Yorkshire. It is part of the ‘avoidable employee harm’ programme which was introduced to the region by the West Yorkshire Mental Health and Wellbeing Hub, in collaboration with colleagues from NHS Wales.

One person described the process of being taken through a disciplinary process like being “plunged into a black hole”, underlining the sense of isolation and abandonment that employees can often experience. The guide includes the main questions that employees often ask during the process and answers them in a clear and accessible way, so that individuals can know what to expect.

“We know that the investigation process can be complex for everyone involved,” said Sonya Robertshaw, Strategic Workforce lead for the West Yorkshire Mental Health, Learning Disability and Autism collaborative within the Partnership. “However, for the person at the centre, it is very often the worst experience of their working lives, because of the emotional, social and psychological impact.”

The guide was developed with input from occupational health, wellbeing and HR colleagues as well as employees who had previously been taken through a process.

The ‘avoidable employee harm’ programme, has seen over 200 staff from 15 organisations attend the ‘Employee Investigation: Looking after your people and the process’ training, established 20 train-the-trainers and built a cross-sector peer network. The programme was adapted for local application, including new intersectionality content, co-created from lived experience and expert-input.

Andrew Cooper, head of the avoidable employee harm programme in NHS Wales, said: “It’s been great to work with colleagues from across the West Yorkshire region and experience their passion and commitment to make a real change in this area of HR practice. The guide will enable colleagues to take the work further by taking a people-centred approach and supporting individuals through what we know to be an extremely demanding process.”

The project has been shortlisted for the Healthcare People Management Association (HPMA) Awards 2025 and has also secured NHS Charities Together Funding to take forward the work at scale.
